What is this about?

If you received an email or letter from Northeast Rehabilitation Hospital Network (“NRHN”) informing you that your sensitive personal and protected health information was compromised in a data breach, you may be entitled to compensation. NRHN recently disclosed that it was the victim of a data breach that compromised individuals’ sensitive personal and protected health information.

What Happened?

On May 22, 2024, NRHN became aware of a security incident on its IT Network. NRHN launched an investigation to determine the nature and scope of the incident. The investigation determined that an unauthorized third-party gained access to some of NRHN’s systems between May 13, 2024 and May 22, 2024 and viewed or copied certain sensitive information.

NRHN then launched a comprehensive review of the incident to identify the exact type of information compromised as well as identify the individuals impacted by the incident. The review was completed on November 27, 2024. On January 6, 2025, NRHN issued a public disclosure and started sending out notice letters and emails to affected individuals.

What Information Was Impacted?

Upon information and belief, the following sensitive information may have been compromised in the data breach:

• Name,
• Contact information,
• Social Security numbers,
• Patient identification numbers,
• Medical record numbers,
• Medical information,
• Treatment information,
• Diagnosis information,
• Health insurance information,
• Driver’s license/ state identification numbers,
• Financial account information, and
• Dates of birth.
