What is this about?

If you received an email or letter from HCF Management, Inc. (“HCF”) informing you that your sensitive personal information and protected health information was compromised in a data breach, you may be entitled to compensation. HCF recently disclosed that it was the victim of a data breach that compromised individuals’ sensitive personal and protected health information.

What Happened?

On October 3, 2024, HCF became aware of a security incident on its IT Network. HCF launched an investigation with the assistance of third-party cyber security experts to determine the nature and scope of the incident. The investigation determined that an unauthorized third-party gained access to HCF’s IT Network on September 17, 2024, and accessed/acquired sensitive personal and protected health information of the individuals.

HCF then launched a comprehensive review of the incident to identify the exact type of information compromised as well identify the individuals affected in the incident. On January 9, 2025, HCF issued a public disclosure and started sending out notice letters and emails to affected individuals.

What Information Was Impacted?

Upon information and belief, the following sensitive personal information may have been compromised in the data breach incident:

• Name,
• Address,
• Phone number,
• Date of birth,
• Social Security number,
• Medical information, and
• Health insurance information
