What is this about?

If you received an email or letter from Lifetime Psychiatry, LLC (“Lifetime”) informing you that your sensitive personal and protected health information was compromised in a data breach, you may be entitled to compensation. Lifetime recently disclosed that it was the victim of a data breach that compromised individuals’ sensitive personal and protected health information.

What Happened?

Lifetime recently became aware of a security incident on its IT Network. Lifetime launched an investigation to determine the nature and scope of the incident. On October 31, 2024, the investigation determined that between September 25, 2024, and September 27, 2024, an unauthorized third-party gained access to employee email account and accessed/acquired certain sensitive personal and protected health information. On December 31, 2024, Lifetime issued a public disclosure and started sending out notice letters and emails to affected individuals.

What Information Was Impacted?

Upon information and belief, the following types of sensitive personal and protected health information was compromised in the data breach:

• Name,
• Address,
• Telephone number,
• Dates of birth,
• Health insurance information,
• Clinical information,
• Diagnostic information,
• Treatment information,
• Bank account information,
• Credit card data, and
• Social Security number.
