What is this about?

If you received an email or letter from Lexington Diagnostic Center (“LDC”) informing you that your sensitive personal and protected health information was compromised in a data breach, you may be entitled to compensation. LDC recently disclosed that it was the victim of a data breach that compromised thousands of individuals’ sensitive personal and protected health information.

What Happened?

On or about March 16, 2024, LDC became aware of a security incident on its IT Network. LDC launched an investigation with the assistance of third-party cyber security experts to determine the nature and scope of the incident. The investigation determined that an unauthorized third-party gained access to LDC’s IT Network and accessed or removed certain files between February 26, 2024, and March 16, 2024.

LDC then launched a comprehensive review of the incident to identify the exact type of information compromised as well identify the individuals affected in the data breach. On November 26, 2024, a further manual document review determined that sensitive personal and protected health information may have been compromised in the data breach. On December 24, 2024, LDC issued a public disclosure and started sending out notice letters and emails to affected individuals.

What Information Was Impacted?

Upon information and belief, the following types of sensitive personal and protected health information may have been breached:

• Full names,
• Address,
• Phone number,
• Date of birth,
• Social Security number,
• Medical record number,
• Patient account number,
• Health plan beneficiary number(s),
• Geographic location, and
• Medical diagnosis/ medical treatment information.
