What is this about?

If you received an email or letter from Cornerstone Healthcare Group Management Services LLC (“Cornerstone”) informing you that your sensitive personal and protected health information was compromised in a data breach, you may be entitled to compensation. Cornerstone recently disclosed that it was the victim of a data breach that compromised tens of thousands of individuals’ sensitive personal and protected health information.

What Happened?

Recently, Cornerstone became aware of a security incident on its IT Network that occurred in mid-December 2023. Cornerstone launched an investigation with the assistance of third-party cyber security experts to determine the nature and scope of the incident. The investigation confirmed that the data breach occurred on December 19, 2023, and it affected data associated with certain healthcare facilities that Cornerstone supports or previously supported. On May 30, 2024, the investigation also determined that sensitive personal information was among the data which was subject to unauthorized third-party access.

Cornerstone then launched a comprehensive review of the incident to identify the exact type of information compromised as well as identify the individuals compromised in the data breach. On January 7, 2025, Cornerstone issued a public disclosure and started sending out notice letters and emails to affected individuals.

What Information Was Impacted?

Upon information and belief, the following sensitive personal and protected health information may have been compromised in the data breach incident:

• Name,
• Social Security number,
• Date of birth,
• Federal or state ID number,
• Financial account information,
• Credit or debit card information,
• Digital signature,
• Email address and password,
• Username and password,
• Passport number,
• Medical or health information,
• Health insurance information, and
• Other protected health information.
