Are we making progress in healthcare #interoperability? 🏥💻 The latest KLAS Research Arch Collaborative report offers some sobering insights. Based on feedback from over 500,000 clinicians, this comprehensive study sheds light on the current state of health information exchange. Despite government mandates for #EHR interoperability dating back to 2004, clinicians' data integration needs are still unmet. ⛔ Key findings from the #report: 👉 Only 44% of respondents agree their EHR provides expected integration with outside organizations 👉 47% of clinicians can't quickly find important patient information from external sources 👉 Another 47% report having to sift through duplicated data The study, which included responses from over 33,000 #clinicians about their experience with external patient data, underscores the ongoing need for improved interoperability solutions. ⭐ Physicians report interoperability as their top fix request, citing difficulties in accessing and leveraging external patient data within their EHR systems. However, it's not all bad news ☀️ The report identifies 17 organizations where 70% or more of clinicians agree that external integration meets their needs. From these success stories, KLAS compiled five key steps to enhance health data interoperability 1️⃣ Commit to sharing data through governance, resource allocation, and strategic planning 2️⃣ Collaborate closely with core sharing partners 3️⃣ Focus on key health measures for shared populations 4️⃣ Elevate end-user EHR awareness and proficiency through targeted training 5️⃣ Involve EHR vendors to optimize data-sharing settings and standardization One exciting finding is the impact of training on perceived interoperability success. Value-based healthcare (VBHC) represents a transformative approach to the delivery of healthcare services. Unlike traditional fee-for-service models, which prioritize the volume of services rendered, VBHC emphasizes patient outcomes and the overall value of care provided. This model aims to enhance the quality of care, improve patient experiences, and reduce healthcare costs by incentivizing providers to focus on delivering better health outcomes. At the core of VBHC is the principle of aligning incentives with patient outcomes. Providers are rewarded not for the quantity of treatments or procedures they perform but for their ability to improve patient health. This shift encourages healthcare providers to adopt more holistic and patient-centered approaches. They focus on preventive care, early diagnosis, and effective management of chronic conditions, all of which contribute to better health outcomes and reduced long-term costs. VBHC also promotes the use of evidence-based practices. By relying on data and clinical research, providers can implement the most effective treatments and interventions. This data-driven approach ensures that patients receive the highest standard of care tailored to their specific needs. Additionally, it facilitates continuous improvement in care processes, as providers can analyze outcomes and refine their methods accordingly. Moreover, VBHC encourages collaboration among healthcare providers. Integrated care teams, consisting of physicians, nurses, and other healthcare professionals, work together to coordinate and manage patient care. This collaboration helps to eliminate redundancies, reduce errors, and ensure that patients receive comprehensive and cohesive care. Another significant benefit of VBHC is cost efficiency. By prioritizing effective and preventive care, VBHC reduces the need for expensive interventions and hospitalizations. Patients experience fewer complications and recover more quickly, leading to lower overall healthcare costs. Insurance companies and government programs also benefit from reduced expenditure, which can be redirected to other areas of public health. Information Systems Research (ISR) has recently published a new article to the Articles in Advance page:   Article Title "How Hospitals Differentiate Health Information Technology Portfolios for Clinical Care Efficiency: Insights from the HITECH Act"   Authors Jessica Pye (deceased), Arizona State University Arun Rai, Georgia State University John Qi Dong, National Technological University, Singapore   Problem definition We examine how hospitals differentiate their HIT portfolios in response to cost inefficiencies in clinical care processes and HIT policy uncertainty.   Relevance Adapting HIT portfolios to address cost-based shortfalls and policy uncertainty is crucial in shaping service delivery and impacting care costs, making it a key concern for policymakers and hospital decision-makers. Theoretical foundations We combine behavioral theory and institutional theory to explain how hospitals differentiate their HIT portfolios through idiosyncratic learning in response to cost-based performance shortfalls and shifting isomorphic pressures from changes in HIT policy uncertainty.   Methodology We constructed a panel dataset of 11,316 hospital-year observations from 3,319 U.S. hospitals over 2007–2014 to investigate how hospitals’ HIT search differentiation is influenced by cost-based performance shortfalls and policy uncertainty during the phases of the HITECH Act, from conceptualization to enactment to enforcement.   Results Our research provides a key insight: Moderate policy uncertainty, particularly during the HITECH Act's phases, significantly shapes hospitals' search for novel HIT solutions to address performance shortfalls. Hospitals enhance their search differentiation when uncertainty decreases from conceptualization to enactment but reduce it as uncertainty diminishes from enactment to enforcement.   Contributions Our findings surface the utility of combining the behavioral theory of the firm with institutional theory to explain the interdependence among a hospital’s cost-based performance shortfalls, HIT policy uncertainty, and HIT search differentiation. Our healthcare ecosystem relies on seamless communication and information exchange between disparate systems and organizations in times of success and disaster. Establishing redundancy and resiliency in healthcare technology ensures sustained, comprehensive interoperability. This is critical particularly in healthcare, where any system failure can have serious consequences for patient care. There are, of course, Benefits and Challenges associated with disaster recovery when it comes to resiliency, redundancy and interoperability. Let's start with the Benefits: Benefit [1] Data Accessibility & Availability Interoperability in disaster recovery ensures that data remains accessible and available across different systems, enabling timely decision-making, efficient coordination, and effective response efforts. Benefit [2] Collaboration & Coordination Interoperability allows different organizations, such as emergency responders, to share critical information, resources, and expertise, thereby enhancing overall response effectiveness. Benefit [3] Rapid Information Exchange Interoperability enables real-time situational awareness, timely updates, and the ability to make informed decisions based on accurate and up-to-date data between different systems and stakeholders. Benefit [4] Seamless Integration Seamless integration between disparate systems enables smooth data flow, eliminates information silos, and the efficiency and effectiveness of the recovery process. And now, the Challenges: Challenge [1] Technical Compatibility Integrating legacy systems with newer technologies, addressing data format inconsistencies, and ensuring seamless communication can require significant effort and resources. Challenge [2] Data Standardization In disaster recovery scenarios, different organizations often use their own data standards, formats, and terminology. Harmonizing and standardizing these diverse data elements can be complex and time-consuming. Challenge [3] Security and Privacy Concerns Ensuring data confidentiality, integrity, and availability while maintaining privacy rights is a significant challenge without the proper security measures and protocols in place. Challenge [4] Governance and Policy Alignment Aligning policies, procedures, and decision-making frameworks across different organizations can be complex, especially in large-scale disasters involving multiple jurisdictions and diverse stakeholders.
