What is this about?

If you recently received a letter or an email from Community Health Center, Inc. (“CHC”) informing you that your sensitive personal and protected health information was compromised in a data breach, you may be entitled to compensation. CHC recently disclosed that it was the victim of a data breach that exposed more than one million patients’ sensitive personal and protected health information.

What Happened?

On January 2, 2025, CHC discovered a security incident on its IT network. In response, CHC launched an investigation and engaged third-party cybersecurity experts to determine the nature and scope of the incident. Upon investigation CHC determined that an unauthorized third party gained access to the company’s systems and may have taken some data that contained individuals’ sensitive personal and protected health information. CHC conducted a comprehensive review of the impacted data to determine what information was compromised and how many individuals were impacted. On January 30, 2025, CHC filed a notice with the Maine Attorney General’s Office and started sending out notice letters to the affected individuals.

What Information Was Impacted?

Upon information and belief, the following types of sensitive personal and protected information may have been compromised as a result of the data breach:

• Name,
• Date of Birth,
• Address,
• Phone number,
• Email,
• Diagnoses,
• Treatment details,
• Test Results,
• Social Security number, and
• Health insurance information
