What is this about?

If you recently received a letter or an email from Prizm Media, Inc. (“Prizm”) informing you that your sensitive personal and protected health information was compromised in a data breach, you may be entitled to compensation. Prizm recently disclosed that it was the victim of a data breach that exposed individuals’ sensitive personal and protected health information.

What Happened?

On July 3, 2023, Prizm became aware of a security incident involving unauthorized access to an employee’s email account. Upon detection, Prizm launched an investigation with the assistance of third-party cybersecurity experts to determine the nature and scope of the incident. The investigation revealed that an unauthorized third party had access to the affected email account between April 28, 2023, and July 4, 2023. Prizm conducted a comprehensive review of the impacted data to determine what information was compromised and to identify affected individuals. On September 26, 2024, Prizm finalized this review and determined that certain personal information was affected. On January 30, 2025, Prizm Media filed a notice with regulatory authorities and began sending out notice letters to the impacted individuals.

What Information Was Impacted?

Upon information and belief, the following types of sensitive personal and protected health information may have been compromised as a result of the data breach:

• Names,
• Dates of Birth,
• Health Insurance Numbers, and
• Medical Information
