On November 24, 2024, SCOSM became aware of a security incident within its IT network. SCOSM immediately launched an investigation and engaged cybersecurity experts to assess the nature and scope of the unauthorized network access. On December 9, 2024, the investigation determined that some impacted locations contained patient information. After completing the investigation on December 20, 2024, SCOSM conducted a comprehensive review of the compromised files to confirm the specific information affected. The review concluded on January 29, 2025, at which time SCOSM determined that patient information was involved and began notifying affected individuals. On January 30, 2025, SCOSM filed a notice with the U.S. Department of Health and Human Services Office for Civil Rights (OCR).

  • Name,
  • Social Security numbers,
  • Driver’s license or state ID numbers, or other identifying numbers,
  • Health insurance data (including health plans/policies, insurance companies, member/group ID numbers, and Medicaid-Medicare government payor ID numbers),
  • Health data (including medical record numbers, doctors, diagnoses, medications, test results, images, care, and treatment), and
  • Billing, insurance claims, and payment data (including claim numbers, account numbers, billing codes, and balances)
