What is this about?

If you recently received a letter or an email from Hillcrest Convalescent Center and Hillcrest Raleigh at Crabtree Valley (collectively "Hillcrest") informing you that your sensitive personal and protected health information was compromised in a data breach, you may be entitled to compensation. Hillcrest recently disclosed that it was the victim of a data breach that exposed individuals’ sensitive personal and protected health information.

What Happened?

On June 27, 2024, Hillcrest became aware of a security incident on its network. Upon detection, Hillcrest launched an investigation with the assistance of third-party cybersecurity experts to determine the nature and scope of the incident. The investigation determined that an unauthorized third party gained access to certain Hillcrest data. Hillcrest conducted a comprehensive review of the impacted data to determine what information was compromised and identified affected individuals. On February 13, 2025, Hillcrest identified persons whose sensitive data was included in the impacted data. On March 4, 2025, Hillcrest filed a notice with the Attorney General of California and started sending out notice letters to the impacted individuals.

What Information Was Impacted?

Upon information and belief, the following types of sensitive personal and protected health information may have been compromised:

• Names,
• Social Security numbers,
• Address,
• Date of birth,
• Driver’s license number,
• Government issued ID number (e.g., passport, state ID card),
• Financial information (e.g., account number, credit or debit card number),
• Medical information (e.g., patient data, treatment information, health care provider information), and
• Health insurance information
